Nine Everyday Prescriptions Quietly Undermining Your Levothyroxine — and How to Outsmart Them

You take your levothyroxine every morning, on schedule, exactly as directed. Yet your latest lab work shows your TSH creeping upward, and your provider is talking about a dose increase. Before anyone touches your prescription, it's worth auditing the rest of your medicine cabinet — and your breakfast table. A surprising number of everyday medications and supplements can bind to levothyroxine in the gut or speed its passage out of the body, leaving you functionally under-treated even though you never missed a dose.

Below are nine of the most common culprits, why they interfere, and the timing strategies pharmacists recommend.

1. Calcium Supplements and Antacid Chews

Calcium carbonate — found in supplements and popular chewable antacids — binds levothyroxine in the intestinal tract, forming a complex your body cannot absorb. Timing rule: Separate by at least 4 hours. If you take your thyroid pill at 7 a.m., hold calcium until lunch.

2. Iron Supplements

Iron is perhaps the most notorious absorber of levothyroxine; studies show co-administration can meaningfully reduce thyroid hormone levels. Many multivitamins contain iron, so read labels carefully. Timing rule: Same guidance — 4 hours apart at minimum.

3. Proton Pump Inhibitors (PPIs)

Drugs like omeprazole and pantoprazole reduce stomach acid, and levothyroxine absorbs best in an acidic environment. The effect is usually modest but can matter in patients already on the edge of adequate dosing. What to ask: Whether your provider wants to monitor TSH more closely after starting or stopping a PPI.

4. H2 Blockers

Famotidine and similar acid reducers pose the same theoretical issue as PPIs, though typically to a lesser degree. Consistency matters more than avoidance here — take them the same way every day so your labs reflect a stable routine.

5. Coffee

Yes, your morning cup counts. Research has shown that coffee significantly reduces levothyroxine absorption when taken simultaneously, an effect documented with tablet formulations. Timing rule: Take levothyroxine with plain water and wait at least 30–60 minutes before your first sip.

6. Fiber Supplements and High-Fiber Meals

Psyllium, bran, and fiber-fortified foods can trap levothyroxine and carry it through the gut unabsorbed. Timing rule: Keep fiber supplements at least 2–4 hours away from your thyroid dose, and be mindful of very high-fiber breakfasts.

7. Soy Products

Soy-based formulas, protein shakes, and infant soy formula can blunt absorption. The interference is real but manageable. Timing rule: Avoid soy in the hour or two surrounding your dose, and keep your soy intake consistent so your dose can be interpreted against a stable diet.

8. Bile Acid Sequestrants and Cholesterol Binders

Medications such as cholestyramine and colesevelam — used for high cholesterol or, occasionally, for digestive conditions — are powerful binders. They can latch onto levothyroxine before it's absorbed at all. What to ask: Your prescriber may need to stagger these drugs by 4 hours or more, or adjust your thyroid dose with careful lab monitoring.

9. Certain Seizure Medications and Antidepressants

Drugs like carbamazepine, phenytoin, and sertraline don't block absorption in the gut; instead, they may accelerate the clearance of thyroid hormone from the bloodstream. This is a different mechanism but the same net effect: your usual dose may no longer suffice. What to ask: For a TSH check several weeks after starting, stopping, or changing the dose of any of these medications.

A Simple Framework to Remember

The Bottom Line

Interactions don't mean you must give up calcium, coffee, or cholesterol treatment. They mean your dosing schedule needs structure. A brief conversation with your pharmacist — the most underused resource in American healthcare — can map every pill you take onto a timeline that lets your levothyroxine do its job. Bring your full medication and supplement list to your next visit; it may be the most productive five minutes of your appointment.
